Medical Home
At Holliston Pediatric Group, we work hard to create a care environment that you will consider your Medical Home. As part of our Medical Home quality initiative we have a Medical Home Care Coordinator, Teresa Breakell, who is available to all providers to help coordinate care for all of our patients, especially those with special needs and/or chronic medical conditions. We aim to be available to all of our patients for any physical or mental health care need, and we hope that you will consider us your medical home.
Behavioral Health
At Holliston Pediatric Group, we want to recognize and support our patients’ mental health needs. Our Social Workers, Donna Jansky and Lindsey Forman, have a combined 50 years of experience working with children and families. As part of the HPG team they consult with the providers and medical staff about the most effective ways to provide clinical care to patient and families who may be experiencing difficulties.
Nutrition
Holliston Pediatric Group offers nutrition services at our Milford location located at 321 Fortune Blvd Ste, 108 with Kimberly Minogue, MS, RD, LDN/Owner of Birchwood Nutrition Group Inc.
Prenatal Consults
Congratulations on the upcoming arrival of your baby!
This is an exciting time for new parents to be! Even though this time is filled with happiness and joy, most parents have many questions surrounding the care and responsibilities of raising a child.
We encourage both expectant and pre-adoptive parents to schedule a prenatal visit. These visits provide an excellent opportunity to meet your baby’s future pediatrician, visit the office and discuss the many questions you may have at this time.
