Policies

Vaccine Policy: We firmly believe in the safety of childhood vaccines, and we believe that immunizing your child according to the schedule recommended by the American Academy of Pediatrics (AAP) and the Center for Disease Control (CDC) is the best way to prevent serious illness and save lives.

Unfortunately, families who refuse to vaccinate their children according to the AAP and CDC recommended schedule may be asked to leave the practice.

Payment Policy: Our fees for medical care are usual and customary for this area and vary with the type of service your child receives. While HPG is contracted with most of the major health insurance carriers, it is your responsibility to understand and comply with the terms of the contract between you and your insurance carrier (we are not part of that contract). Some of the services we provide may not be covered by your health insurance and payment for these services are ultimately your responsibility. 

Insurance/Billing Policy: Due to all of the recent changes in the health care industry, our billing department receives many calls from parents with questions regarding their bills and charges incurred during a “routine well visit” or “office sick visit.” 

Adolescent Confidentiality Policy: Adolescence is a time of transition toward adulthood and we will support this healthy transition in our office. During our adolescent well visits, we will have part of each visit alone with our teens. When teens share something with us that they ask to remain confidential, we will honor that request, unless they plan to harm themselves or someone else.

Privacy Policy: We take your child’s protected health information very seriously. This information is highly guarded and respected.


Forms:

We require 5-7 business days for referral requests.  We will accept referral requests on Saturday mornings. However, the referral will not be processed until the next business day.

We require 2 business days for a second copy of a school form.

We require 2 weeks notice for a specialized camp form.

We require 3 days notice Mon-Friday for a prescription refill.  
Please do not leave any prescription refill requests on voicemail during the weekend.
